Team Primary Care Clinician, Office of Developmental Primary Care

The Office of Developmental Primary Care (ODPC) is a program in the Department of Family and Community Medicine at the University of California, San Francisco. We are dedicated to building the capacity of the health care system to serve transition age youth and adults with developmental disabilities through CART: Clinical Service, Advocacy, Research and Training. Our goal is that all people with developmental disabilities have access to the health care services they need to maximize their wellness and function. We believe that health care for people with developmental disabilities is interdisciplinary team‐based care with patients and caregivers at the center of the team.

This position will combine home‐based primary care for a small panel of complex patients; clinical consultation, and other related training and advocacy activities. This is an excellent opportunity for someone interested in developing deep expertise in care for youth and adults with developmental disabilities and piloting innovative models for health service delivery for vulnerable populations. It can be combined with other teaching, research, or clinic‐based work at UCSF.

Job Duties:
Home‐based, primary care for a small panel of medically fragile and behaviorally complex adults with developmental disabilities. Participation as a member of a mobile consult team (primary care, psychiatry, nursing, therapists) who will provide in person, email, phone and telemedicine consultation and assessments across Northern California. Population‐based medicine; quality improvement; health service delivery innovation; program evaluation; quality improvement; and training projects. Job hours vary based on patient need. After‐hours work and travel in the Bay Area and Northern California is required.

Qualifications:
Successful applicants must be a physician board certified or eligible in Family Medicine, Internal Medicine or Med/Peds or a Masters or Doctoral level Nurse Practitioner with experience providing primary care to adults at the time of hire. In addition, the successful applicant will have strong clinical and verbal and written communication skills. Specific expertise in primary care for adults with developmental disabilities is desirable, but not essential. Applicants should have some previous personal or professional exposure to people with complex disabilities. Mentorship will be provided. Ideal candidates will be self‐motivated, flexible, and willing to learn and assume increasing responsibility. Language and/or cultural skills are desired to serve Northern California’s diverse populations. Ability to drive and use mobile technology such as laptops and cell phone is required. This position can be full‐time, part‐time or, for internal candidates, combined with other faculty duties at UCSF.
